Every full dose on a Mounjaro KwikPen is 60 clicks, and that is true of all six strengths — the dial is calibrated the same way in each. What changes between pens is how much tirzepatide one click delivers.

So the arithmetic for any part-dose is one line:

clicks = (your dose ÷ the pen's strength) × 60

On a 15 mg pen, 2 mg is 2 ÷ 15 × 60 = 8 clicks. On a 10 mg pen, the same 2 mg is 12 clicks. The dose did not change; the pen did.

Counting without losing track

The dial gives one click per increment and no display, so the count is yours to keep. Three things make it survivable:

  • Count out loud, in threes or fives. Twenty clicks counted singly is twenty chances to lose the thread; four groups of five is four.
  • Write the number down before you inject, not after. Once the dose is delivered there is nothing left to check it against.
  • Keep the running total per pen. A KwikPen holds four full doses — 240 clicks in total — so part-doses drain it at a rate you have to track yourself to know when it is empty.

When the dose does not land on a click

Clicks are indivisible, so a dose only comes out exactly when it divides cleanly by what one click delivers. On a 15 mg pen every click is 0.25 mg, so 2 mg, 2.25 mg and 2.5 mg are all reachable and 2.1 mg is not — it falls between click 8 and click 9. Pens with finer increments (a 5 mg pen clicks in 0.083 mg) reach more values, but none of them reach all of them.

The KwikPen is licensed to deliver its full labelled dose. Dialling a part-dose by counting clicks is off-label use of the device, and the arithmetic above describes how the pen is calibrated rather than recommending any particular amount. If you are counting clicks, it should be because the person who prescribed your medication asked you to.

0.25 mg. Divide the pen strength by 60: a 15 mg pen delivers 15 ÷ 60 = 0.25 mg per click, a 10 mg pen 0.167 mg, a 5 mg pen 0.083 mg.
How many clicks is 2 mg from a 15 mg pen?
Eight. The formula is (dose ÷ pen strength) × 60, so 2 ÷ 15 × 60 = 8 clicks. Doses that do not divide cleanly land between clicks and cannot be dialled exactly.
Is counting clicks approved by the manufacturer?

The multi-dose KwikPen is the UK and European presentation. In the United States, tirzepatide is supplied in single-dose autoinjectors that have no dial and no clicks, so there is nothing to count.
